Routine appointments

Patients are advised wherever possible to use our online request system which you can access below.

You will be asked to provide information about your need for an appointment and our team will aim to contact you within 24 hours.

If you are unable to go online, please call our Reception Team who will take you through the online form.

All requests are processed in the same way.

Sometimes you may not need an appointment with a member of our team for your problem and will instead be signposted to the appropriate service for your need.

Cancelling your appointment

If you have an appointment and you are unable to attend or wish to cancel, please let us know as soon as you can either by telephone on 01454 323366 or using our online form below.

Extended Access

Working within our current staffing capacity we are doing our best to ensure that you have access to additional appointments outside of standard practice hours.  As a result you may be offered an evening appointment. If this is something you would prefer please ask about availability when booking an appointment.

 

Home Visits

If possible please try to telephone reception before 10:30 if you require a home visit.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.

A health professional from the Yate locality community health team may carry out a home visit request if you are referred by your GP.
