Travel Vaccinations
If you require any vaccinations relating to foreign travel please complete the travel health questionnaire below.
This will provide us with information we need to determine what vaccinations are required. Once our nurse has reviewed this you will be contacted by one of our reception team to arrange an appointment to have the necessary vaccinations. It is important to complete and submit the travel health questionnaire form at least 6 weeks before you travel to allow sufficient time to receive the vaccinations (at least 2 weeks before you travel).
It is also worth noting that these vaccines have to be ordered as we do not we keep them in stock.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
